On April 1, 2025, the Trump administration withheld Title X family-planning grants from 16 grantees in 23 states, disrupting funding for birth control, cancer screenings, STI testing, and other healthcare services to 879 clinics nationwide. The Washington Post estimates that 66 million dollars were paused, resulting in 834,000 women losing access to these healthcare services. That represents 30% of current Title X patients.

Why it matters

Title X doesn’t fund abortion. It funds basic, preventive care that low-income patients—mostly women—use every day. When these grants are frozen, clinics cut hours, reduce services, or close sites, and patients get longer waits or no local option at all. That is why grantees and other healthcare advocates moved quickly in court to challenge the freeze.

Background

Congress created Title X in 1970 to support affordable family planning and related preventive health services. It is the nation’s only dedicated family planning program that has received long-standing bipartisan support. According to the Center for Reproductive Rights, the program serves millions of Americans each year, ensuring that low-income and marginalized communities across the country can access vital family planning and preventive care.

President Trump changed Title X funding during his first term, which led to 844,083 women losing access to these family-planning services in 2019 alone. Despite efforts to reverse this decision, the Title X provider network never fully recovered - in 2023, the program served 2.8 million people compared to 4 million in 2017.

As of October 2025, the administration’s April freeze on Title X grants hasn’t been fully reversed. Several grantees have had funds restored, while others report ongoing gaps. Legal challenges led by NFPRHA and Planned Parenthood affiliates are active, but no blanket court order has restored funding to everyone.
